To complete Grades 9 - 12 registration, please find all documents in the designated folders below. If you have any trouble accessing these documents please let us know. Please note the Affidavit Residency Form - is needed ONLY if you live with a Burlington resident and you do not own or lease your residence.
The following documents are REQUIRED before a registration is complete:
- Original birth certificate
- 2 proofs of residency
- Most current physical/immunizations
[ Translation support services available if needed ]
- Custodial or court order (if applicable)
* We also request a copy of your child's most recent report card,
as well as any other educational reports if applicable (IEP or 504).
